JOB SUMMARY : Provide clerical and operational support to the PACE Therapy Department to ensure efficient coordination of participant care. This position plays a vital role in maintaining accurate documentation, coordinating communication between the rehabilitation team and participants, and supporting the delivery of high-quality, person-centered rehabilitative services consistent with the PACE model of care.
QUALIFICATIONS :
High school graduate or equivalent.
CNA license preferred.
Rehab tech experience preferred.
Pleasant outgoing personality.
Ability to interact in a supportive manner with people of all backgrounds.
Ability to work under pressure and handle conflicting demands.
Knowledge of medical terminology required.
Computer knowledge – Excel and Word - required.
Electronic medical record experience preferred.
Excellent telephone skills and etiquette.
Must be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
Criminal history records check at hire.
Drug screen at hire.
JOB RESPONSIBILITIES :
- Hold primary responsibility for scheduling and check-in of therapy appointments including follow-ups, cancellations and reschedules.
- Conduct well-checks via telephone to ensure participant well-being and identify potential needs requiring therapy intervention.
- Obtain necessary signatures for authorization, releases, or issuance of durable medical equipment (DME).
- Track therapy orders, frequencies, and completion status in accordance with the overall compliance plan.
- Collect and input data to support tracking of functional outcomes and quality improvement initiatives.
- Order and track therapy supplies and DME; maintain accurate records of previously issued and newly provided DME.
- Maintain referral logs, ensuring initiation, follow-up, and closure of therapy referrals as needed.
- Make outgoing and receive incoming calls related to scheduling, coordination, and participant needs.
- Stock, clean, and maintain the therapy area and equipment to ensure safety and infection control standards.
- Assist with organization of departmental materials, including handouts and educational resources.
- Provide supportive assistance to licensed therapists during participant care, under supervision and within defined scope.
- Is courteous, polite and helpful to participants, staff, and the public.
- Participates in staff meetings and in-service training sessions as required.
- Performs other duties as required.
