NYC Health + Hospitals / Bellevue is America's oldest public hospital, established in 1736. Bellevue's clinical centers of excellence include : Emergency Medicine and Trauma Care; Cardiovascular Services; Designated Regional Perinatal Center and Neonatal Intensive Care Unit (ICU); Comprehensive Children's Psychiatric Emergency Program; and Cancer Services.

Work Shifts

9 : 00 A.M - 5 : 00 P.M

Duties & Responsibilities

Purpose of Position :

This class of positions encompasses work of varying degrees of difficulty and responsibility in the performance of liaison functions between City agencies, local residents, and community organizations, in order to : provide assistance, obtain participation and support for housing development and community development and implement public service programs. Performs tasks in the field or in an office, as assigned. Supervises staff. All personnel perform related work.

Under general supervision, performs duties involving liaison activities between City agencies, local residents, and community organizations.

Examples of Typical Tasks :

At a higher level, performs the duties of Assignment Level I :

1. Interviews local residents, landlords, government personnel, business people and youths to solicit

support for community development programs, to ascertain needs of communities and to determine

courses of action to meet those needs which are acceptable to the communities.

2. Interviews staff and representatives of governmental, educational, and social agencies and unions for the purposes of securing information for and transmitting information from community organizations and residents.

3. Assists in the preparation and distribution of informational materials relating to the programs, objectives, and activities of both communities and educational institutions.

4. Assists in the training of subordinate staff.

5. Makes written and oral reports as required.

6. Keeps records related to programs, activities, objectives, etc.

In addition to performing duties of Assignment Level I at a higher level, as described above, also performs the following :

1. Directs staff or works as a team with other community liaison workers in the performance of liaison activities; may supervise or coordinate field office operations or operational units.

2. Consults and maintains liaison with residents, landlords, representatives of community organizations, students, parents, staff of schools, unions and other public and private agencies.

3. Explains programs and policies to community groups and to community liaison contacts and interprets and relays information from the community.

4. Participates in the planning, organization and implementation of programs involving community

participation. Collects, evaluates, and reports information regarding community needs and objectives; communicates findings to appropriate agencies for necessary action. Assists in the conduct of outreach programs.

5. Assists in mediating problems between the community and schools, hospitals, and other City agencies for residents and community groups in the City.

6. Works as a member of an interdisciplinary team assisting in providing supportive therapy and making referrals to City agencies, social service agencies, community organizations and other organizations that provide health and social services.

7. Counsels community residents, property owners, local businesses and groups on their rights and obligations and on available community and governmental resources.

8. Makes recommendations on effective mobilization for participation in community development and program feasibility and improvement.

9. Recommends policies, procedures, and priorities for City agencies in dealing with schools, residents, and community groups.

10. Interviews community residents and assists them in completing forms; completes other relevant reports and forms and performs related clerical work.

Minimum Qualifications

For Appointment to Level I

1. One year of full-time experience in a government agency or community organization in providing community services to the public or in assisting members of the community in obtaining community services; or

2. Completion of an approved six month training program of combined classroom training and on-the-job experience in community liaison work, plus six months of full-time experience as described in 1 above.

For Appointment to Level II

In addition to meeting the qualification requirements of Assignment Level I, appointment to Level II requires :

1. Two additional years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or

2. Education and / or experience equivalent to 1 above. Study at an accredited college in sociology, psychology or other behavioral science may be substituted on the basis of 30 semester credits for each year of the experience described above. However, all persons must have at least one year of the full-time experience described above.

For Appointment to Level III

In addition to meeting the qualification requirements of Assignment Level II, appointment to Level III requires :

1. One additional year of full-time experience as described in 1 above.

For Appointment to Level IV

In addition to meeting the qualification requirements for Assignment Level III, appointment to Level IV requires :

1. One additional year of full-time experience.

Department Preferences

  • Experience working with patients experiencing homelessness, individuals with chronic health conditions, and / or those with personal lived experience in similar contexts. Ability to address complex health, social, and service-related needs, and to contribute to the development of individualized, patient-centered, goal-directed care plans.
  • Proven ability to assist patients in connecting with primary care providers, specialty care, community resources, and mental health services. Familiarity with supporting patients in attending medical appointments, re-engaging after missed visits, filling prescriptions, and navigating social needs such as food insecurity, housing, legal aid, transportation, job readiness, and insurance enrollment.
  • Experience conducting home and community-based visits to assess patient needs in diverse living environments, including street or shelter settings. Comfort working off-site in mobile clinic settings including walking around the community and engaging patients through various outreach methods including in-person, telephone, and email.
  • Ability to provide in-person support to patients during medical appointments and hospital stays as needed. Strong communication skills to collaborate effectively with internal care teams and external service providers through various channels, including electronic health record systems.
