Join the Adult Home Health team!

Our Community Health Nurses provide skilled, compassionate care to patients in their homeshelping individuals remain where they feel most comfortable . We work to the top of our licensure and collaborate closely within a comprehensive, interdisciplinary team that includes :

Registered Nurses (RNs)

Licensed Practical Nurses (LPNs)

Licensed Nursing Assistants (LNAs)

Physical Therapists (PTs)

Speech-Language Pathologists (SLPs)

Social Workers

Dedicated Administrative Support

Together, we ensure each patient receives the highest level of personalized, at-home care.

Qualifications

1+ year of home health or public health nursing experience preferred, OR a combination of education and experience that provides comparable skills

Home care experience is helpful but not requiredwe will happily train motivated candidates who are eager to learn

Incentives

$10,000 Sign-On Bonus for external applicants (prorated for part-time roles)

Mileage reimbursed at federal gas rates

On-call : Not required

Requirements :

Current RN licensure recognized by the State of Vermont required.

Appropriate experience in specific clinical area. Varies by unit.
