At Home Matters Caregiving, our Licensed Practical Nurses (LPNs) are trusted leaders and essential members of our care team. LPNs bridge the gap between clinical excellence and compassionate leadership — guiding caregivers, supporting families, and ensuring every client receives care rooted in dignity, safety, and purpose. As part of our clinical team, the LPN plays a hands-on role in training, delegation, and coaching. This position requires both clinical skill and emotional intelligence, leading with empathy during challenging conversations and ensuring care plans reflect the heart of our Purpose Driven Care (PDC) and Nurse Guided Care philosophies.
Qualifications / Education :
Current, unencumbered Colorado LPN license.
Two years of experience in home care, home health, or related healthcare settings.
Demonstrated experience in delegation, training, or caregiver supervision.
Strong verbal, written, and documentation skills.
Basic computer proficiency (EMR systems, email, and charting).
Able to lift up to 50 lbs using safe ergonomic techniques.
Valid driver’s license and reliable transportation.
Knowledge of Colorado in-home care rules and Nurse Practice Act.
Job Duties :
Conduct nursing assessments and collaborate with the Clinical Director to develop and update individualized care plans.
Delegate and re-delegate nursing tasks (e.g., wound care, insulin injections, CBG checks, Lovenox administration) per Colorado Nurse Practice Act.
Train and supervise caregivers on delegated tasks, safety procedures, and documentation expectations.
Perform routine supervisory visits, 60-day, and 90-day reviews to ensure quality and compliance.
Communicate with physicians, families, and caregivers regarding care updates, medication changes, and client conditions.
Oversee medi-set refills, medication reconciliation, and MAR reviews to maintain accuracy and safety.
Partner with hospice and external agencies to coordinate care and uphold Home Matters standards.
Support caregivers through coaching and mentorship, addressing performance concerns with professionalism and compassion.
Maintain up-to-date knowledge of home care regulations, delegation standards, and clinical best practices.
Documentation & Communication
Accurately document all delegations, visits, and updates in the EMR.
Keep care plans and medication records current and compliant.
Report client changes promptly to the Clinical Director.
Communicate with clarity, empathy, and professionalism at all times.
Training & Competency
Complete required orientation, annual training, and continuing education.
Maintain current CPR, infection-control, and HIPAA certifications.
Continue developing leadership and delegation skills through ongoing learning.
