Job Summary
OdysseyHouse Louisiana, Inc. is seeking a full-time Case Manager. Case Managerposition determines eligibility for guests to participate in the agency's LowBarrier Shelter (LBS). The Case Manager will assess guests' needs to create aneffective housing plan. Case Managerswill meet with guests regularly to ensure they are reporting to referredcommunity resources pertinent to housing. Case Managers will use LA Medicaidsystem and assist guests with applying and / or recertifying as needed. Accessing and entering information into ViaLink'sHomeless Management Information System (HMIS) database, case note documentation,discharge planning and monitoring programmatic activities pertinent to theguest's progress towards obtaining housing will be the primary responsibilityof the Case Manager. In some cases, theCase Manager may provide guests with transportation to medical appointments byutilizing Medicaid transportation or an agency vehicle when necessary. CaseManagers will prepare weekly, monthly, quarterly, annual reports, and attendall department me as required.
Responsibilitiesand Duties
It is theduty and responsibility of the Case Manager to :
- document services and providecase management to approximately thirty or more guest(s) as assigned;
- Case Management will involvescreening, intake, assessment, individualized housing plan development,monitoring, linkage to appropriate community resources, follow up, appropriatetransition to housing, etc.;
- assist guest(s) withobtaining vital records, ID cards, Driver's License and other documentation;
- establishingpartnerships with housing agencies
- assist guest with signinglease agreements when necessary
- assess guest insurance statusand follow agencies policy and procedures to obtain and maintain Louisiana Medicaidstatus;
- enter guest's informationinto the appropriate agency's database(s) in a timely matter and update asneeded;
- prepare and submit reports toProject Manager in a timely manner;
- maintain up-to-dateinformation regarding guest occupancy, move outs, and vacancies
- utilize MotivationalInterviewing (MI), a client-centered approach for eliciting behavior changes byguiding participants to explore and resolve ambivalence;
- ensure that guest(s) arereferred to and / or aware of available vocational, educational, housing, andsocial services in the community;
- attend required meetings andtrainings;
- verify the results of theassistance provided to close the case, as per the approval of the ProgramManager;
- provide crisis managementwhen indicated by guest(s) needs;
- complete any Incident Reportsprior to the end of scheduled shift as required by Incident Policy andProcedure;
- maintain guests' licensingrequirements;
- oversee daily guestactivities for guest(s) in the LBS; and
- perform other duties asassigned.
Qualifications and Skills
Required
Be over 18 years of ageExcellent communication, organizational,and interviewing skillsAbility to work with unhousedguests of diverse backgrounds and disabilities including physical, mental,substance abuse, immunodeficiencies, etc.B. A. Degree in socialwork / behavioral science or three years relevant work experienceValid Louisiana Driver'sLicense with cleaning driving recordAbility to navigate eightpassenger vanPreferred
