JOB SUMMARY :

Provides personal care and / or homemaker services to individuals in their place of residence and assists them in maintaining their home.

QUALIFICATIONS :

• Meets competency requirements.
• Speaks, writes, reads, and comprehends oral and written instruction.
• Able to lift, push, and pull light to moderate loads.
• Valid Driver's License
• One year personal care related experience or State certification.

RESPONSIBILITIES (including, but not limited to) :

PHYSICAL DEMANDS :

Able to lift, move, and pull light to heavy loads. May do heavy lifting, pushing, or pulling, up to 25 pounds.

LICENSES and CERTIFICATIONS

1. Current Driver's License

2. Current Automobile Liability Insurance According to Company Policy & Procedure.

3. State Certificate if applicable.

4. CPR and First Aid Preferred
