Shift : MondayFriday days with potential weekend rotation and on call

Job Description :

Sunbelt Staffing is hiring an experienced Hospice Home Health RN to provide quality hospice care for patients in the Portland, OR area. This role offers a weekday schedule with additional weekend and on call coverage as needed. Candidates must be proficient in Homecare Homebase and have strong clinical skills to support patients and families during end-of-life care.

Key Responsibilities :

Deliver compassionate hospice care and nursing support in patients homes

Monitor patient conditions and adjust care plans accordingly

Work with the interdisciplinary team to ensure coordinated patient care

Provide education to families on symptom management and emotional support

Complete timely and accurate documentation in Homecare Homebase

Qualifications :

Active Oregon RN license

Minimum 2 years of nursing experience, with at least 1 year in hospice home health

Experience with Homecare Homebase (required)

Strong clinical assessment and interpersonal skills

Ability to work independently and manage a patient caseload

Reliable vehicle, valid drivers license, and auto insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
