Geriatrics Nurse Practitioner

Nurse Practitioner / Physician Assistant Remote (Michigan Residents Only)

This opportunity is offered through Medix , a national healthcare staffing agency, in partnership with one of our top clients dedicated to improving access to quality healthcare in rural communities .

We are seeking a fully remote Nurse Practitioner or Physician Assistant who is passionate about rural healthcare and providing compassionate, patient-centered care for adult and geriatric populations . The ideal candidate has experience with value-based care models , risk adjustment factors (RAFs) , and chronic and complex care management .

Candidates must live in Michigan and be credentialed with BCBS and Aetna in the state to qualify.

Quick Summary of the Position

• Fully remote position
• Working within a value-based care model focused on adult and geriatric patients
• Salary range : $110,000 $130,000 per year
• W2, full-time position with benefits
• Schedule : Monday through Friday, 8 : 00 AM 5 : 00 PM (includes 2 hours of built-in administrative time)
• Providing care for patients in rural communities
• patients per day
• Start date : As soon as credentialed
