Work Schedule : Monday - Friday, 8am - 4pm. Some weekend / evening work may be required

JOB SUMMARY :
The Case Manager II, Programs will assist in the design and delivery of program activities for person with mental, physical, and / or intellectual / developmental disabilities. This position will be responsible for continuous support to the Employment and Community based programs. ESSENTIAL FUNCTIONS :
The Case Manager II, Programs will perform the following duties personally or through delegation :
Supervises a caseload of consumers consisting of all disability areas and encourages the utilization of multiple community agencies serving persons with mental, physical, and / or intellectual / developmental disabilities.
Develops and maintains contact with other agency staff and local service providers to develop specialized services for consumers and their families.
Assigns, guides, coaches, and trains subordinates in alignment with Community Employment and Day Programs, and Goodwill policy and procedures. Provides information concerning case management practices, administrative rules and regulations, community resources, vendors, and programs.
Serves as a resource to address sensitive consumer needs and / or accommodations.
Develops Individual Services and Supports Plans for each consumer within the programs, implements and monitors plan progress. Adjusts and modifies development plan as needed to meet changing consumer needs.
Participates on the inter-disciplinary team to review each consumer’s case and participates in all inter-disciplinary team meetings for each consumer.
Prepares or dictates case progress notes, social summaries, and other reports concerning the consumer’s case.
Attends staff meetings concerning new or revised policies and procedures.
Collaborates with and receives direct supervision from Manager in making decisions related to consumer services.
Arranges for American Sign Language interpreters, as needed.
Performs any other duties as required or assigned.

Education :
Experience :
Prior experience working with individuals with intellectual or developmental disabilities is preferred.
One (1) year of experience as a case manager and / or administrative positions is preferred.
Other :
It is preferred that the incumbent have excellent written and verbal communication skills and demonstrate the ability to interact with various types of internal and external customers.
It is also preferred that the incumbent have the ability to relate effectively to people with disabilities, their parents, host home providers and guardians, and be empathetic toward their needs.
The incumbent must have the ability to work independently, cooperatively, and to exercise effective planning.
The Case Manager II, Programs must demonstrate flexibility within the framework and pressure of established deadlines.
The incumbent must be able to write clear, concise and accurate reports and plans, submit them in a timely fashion and possess excellent time management skills.
The incumbent must have a working knowledge of community resources, have the ability to program, train and be creative in motivating Participants toward individual success.
The incumbent must also conduct self in a professional manner and maintain a professional appearance that enhances the firm’s reputation.
Overtime may be required as well as some travel to attend meetings and required training. The ability to work occasional weekends and nights for the Residential on-call system is required.
The incumbent will be required and must have the ability to drive for company business
19 years of age (not engaged in passenger transportation),
25 – 70 years of age for all passenger transportation services,
21 years of age for CDL.
Applicants / employee will be required to undergo a Motor Vehicle Record (MVR) check and background check; applicants / employees must disclose all moving traffic violations or vehicle crashes (within the last three (3) years)(Note : not all violations are a disqualification for employment and each case will be reviewed by the insurance carrier); must have valid Colorado State driver license; must also be able to operate company vehicle, and adhere to all Federal, State, and local laws governing vehicle operation.
Applicant / employees using their personal vehicle for company business, must have valid vehicle insurance, valid driver license, and vehicle must be in roadworthy condition.
