Job Description :

Sunbelt Staffing is seeking a Home Health RN for a contract assignment in Columbus, GA. The nurse will complete OASIS documentation, provide skilled home visits, and manage care plans through Homecare Homebase (HCHB). This role requires strong clinical skills, attention to detail, and the ability to work independently in the field.

Key Responsibilities :

Perform patient admissions, recerts, resumptions, and discharges using OASIS

Provide skilled nursing care and health education to patients and families

Document all care accurately in Homecare Homebase (HCHB)

Communicate with physicians and the care team for ongoing patient needs

Ensure compliance with all agency and Joint Commission standards

Qualifications :

Current Georgia RN license

At least 2 years of RN experience, with a minimum of 1 year in home health

OASIS and Homecare Homebase (HCHB) experience required

Valid drivers license, car insurance, and reliable transportation

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
