Schedule / Hours :

• Variable day and afternoon shifts

Responsibilities :

• Review and interpret physician medication orders for appropriateness and safety
• Provide patient counseling and medication education
• Monitor patient drug therapies for effectiveness and adverse reactions
• Collaborate with healthcare team to develop and implement therapeutic plans
• Ensure pliance with hospital policies and regulatory standards
• Maintain accurate documentation and records on pharmaceutical care activities

Qualifications :

• Licensed Pharmacist with experience in clinical pharmacy
