Clinician-RESTORE Family Treatment Team

Position Summary :

Under direct supervision of the RESTORE Program Manager, assists with treatment planning and delivery of clinical and care coordination services of RESTORE. Performs psychosocial assessments, treatment planning / monitoring and discharge planning. Provides counseling / therapy to clients requiring complex intervention. Performs commitment determinations and / or executes orders of involuntary commitments.

Job Responsibilities & Performance Standards :

• Performs job responsibilities with supervision.
• Serves as job expert or organization resource in assigned areas.
• Acts as client advocate to coordinate required services or to resolve crisis.
• Completes family intakes based on interviews with youth, their families, significant others, and appropriate community agencies.
• Counsels' clients in individual, family, and group sessions to help them overcome dependencies, recover from illness, and adjust to life.
• Identifies environmental impediments to client of progress through interviews and review of patient records.
• Monitors, evaluates, and records client progress according to measurable goals described in treatment plan.
• Organizes support groups or counsels' family members to assist them in understanding, dealing with, and supporting the client.
• Performs clinical assessments of the behavioral health needs of families presenting for services.
• Provides case management services related to identified treatment needs.
• Provides training to direct care staff in implementing behavior modification techniques and treatment plans.
• Refers client or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education.
• Designs behavioral or therapeutic interventions for intractable or difficult to manage behavioral health issues.
• In collaboration with the RESTORE Program Manager, determines the necessity for commitment in accordance with accepted standards of clinical practice.
• Conducts psychosocial assessments / evaluations to develop comprehensive case / service plans.
• Provides treatment to individuals, groups, families, and couples using a variety of psychotherapeutic techniques.
• Conducts advanced level psychosocial assessments / evaluations and utilizes information from other service providers, physicians, team members and client / representative to develop comprehensive care / service plans and to use in court depositions, etc.
• Performs crisis intervention services to families served by RESTORE. Consults with Program Manager regarding needs for higher level of care treatment.
• Provides treatment to clients in their homes and other community settings.
• Participates in on-call schedule with ability to be on-call-by phone or face to face as needed-including after hours, weekends, and holidays.
• Documents all interact with youth, family, or collateral contacts-billable and nonbillable-in the electronic health record.
• Maintains the strictest confidentiality of all information related to the child's health care. Obtains a signed release from the family before sharing information with others.

Minimum Qualifications :

Clinician 1 : Master's degree in a related area from an accredited college or university AND eligibility to be licensed in the state of Georgia.

Clinician 2 : Master's degree in a related area from an accredited college or university AND associate licensure in the state of Georgia.

Clinician 3 : Master's degree in a related area from an accredited college or university AND full clinical licensure in the state of Georgia.
