• Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed).\n
• Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists.\n
• Collects and records relevant patient health data in the course of / pre / post hemodialysis treatment.\n
• Develops and maintains a plan of care that describes interventions to attain expected outcomes.\n
• Implements and records interventions to attain expected outcomes in EMR.\n
• Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections.\n
• Occasional travel to other nearby locations.\n
