Job Summary :
This position provides coaching and intensive short term Financial Assistance Case Management to help adults build self-sufficiency and gain economic empowerment. This position works towards the goal of helping the client to end / decrease reliance on crisis assistance for their health, safety and welfare needs.
Minimum Qualifications :
Education : Bachelor’s Degree in Social work or related field.
Experience : Minimum of 6 months experience in case management, engaging hard to reach people, assessment, housing stability, planning and connecting to resources / referrals.
Primary Job Functions :
- Work with the client to create a plan of action that wraps individualized responsive instruction and services around clients.
- Teach concrete life skills
- Conduct one on one visits with the client
- Complete an intake with the client to gather relevant information to enter into Welsky
- Create a Housing Stability Plan that includes information on events or circumstances that led to the current need, barriers, strengths and action steps with target dates
- Provide coaching on Financial Literacy (budgeting / establishing a savings fund / building credit)
- Assist with job searches, interviewing skills, building a resume
- Brainstorm strategies to increase income / decrease expenses
- Review affordable housing options
- Link with educational / vocational resources
- Access JFCS programs / resources and community resources (SNAP / TANF / HUD / Medicaid / ACCESS / Disability)
- Advocate for / accompany clients to appointments
- Help get required documentation for benefits
- Help sign up for virtual school or evaluate vocational options to increase clients employability
- Complete exit interviews with clients
- Collect data on housing stability by doing JEA searches at specific time frames to report back to COJ
- Maintain client files
- Other duties as assigned
