Job descriptionExperience : At least 1,000 hours of experience with geriatric or chronically ill individuals is preferred. Exceptions made for CNAs, HHAs, or those providing companionship / housekeeping only. Certifications : CNA or HHA certifications are a plus but not required. Health & Safety : Current TB test, ability to safely assist with client transfers, and adherence to infection control practices. Transportation : Reliable vehicle, valid driver’s license, and car insurance are required. Background : Successful completion of criminal background checks (both national and state required). Communication : Strong communication skills for interacting with clients, family members, and team members. Care Plan Execution : Implement personalized care plans, ensuring clients receive the care they need. Comprehensive Support : Assist with companionship, meal preparation, housekeeping, transportation, and personal care (bathing, dressing, feeding, etc.). Advocacy & Communication : Report any changes in the client’s condition and advocate for their needs to the Care Manager. Safety & Compliance : Follow all safety protocols and infection control standards, and alert the office if additional supplies are needed. Timekeeping & Documentation : Log hours, tasks, and client updates accurately using our ClearCare Telephony system. Professionalism : Maintain a professional appearance and adhere to company policies. Ability to assist with client transfers using proper techniques and equipment. Reliable transportation to client homes.
