Multi-State Licensed Nurse Practitioner

A company is looking for a Remote Nurse Practitioner to assist patients in navigating their remote care journey.

Key Responsibilities

Oversee monitoring of patient vitals, symptoms, and labs in collaboration with RNs and MDs

Manage treatment plans and medications, focusing on guideline-directed medical therapy for chronic conditions

Lead virtual check-ins with patients to review labs and optimize adherence to treatment plans

Required Qualifications

5+ years of experience treating chronic diseases (T2D, Hypertension, CHF) in a medical setting

Active multi-state compact RN license and Nurse Practitioner (NP) license in specified states

ANCC or AANP certification required

Master's Degree as a Nurse Practitioner with a willingness to expand state licensure

Experience managing CHF with guideline-directed medical therapy (GDMT)
