This role is focused on exemplary customer service, engagement with clients, families, care professionals, and engagement with local healthcare organizations.
Handling incoming client inquiry calls and finding solutionsCompleting home assessments / home visits for new clients, and initiating the plan of careMeeting with clients and introducing care professionalsInterviewing and training care professionalsCommunicating and engaging regularly with clients, their families and care professionals, and local healthcare organizationsCoordinating and partnering with dedicated scheduling teamReviewing care notes and highlighting areas of interestBeing available and providing back-up client care as neededClient Care Coordinator Requirements :
Experience – Experience providing personal (activities of daily living - ADL) care. Office Experience. Customer Service Experience.Education – High school diploma and some college preferred. Medical Assistant (MA) certification and / or CNA preferredSkills – Relationship and exemplary communication skills. Professional, prioritization, time management skills. Effective with computer application technology.Schedule - Availability to contribute to the team 40 hours per week (Generally 8 : 00am-5 : 00pm) Monday through Friday and some weekendsLocation - Must live in or near Brentwood / Franklin, TN .
