Wits Solutions is looking for a Case Manager to join our team supporting our DoD Customer's Case / Care Management requirements.

Responsibilities

  • Coordinate directly with the COR and DSS on the most difficult case files.
  • Responsible for overall quality assurance and timeliness of delivery for all work performed.
  • Facilitate case management initiatives, and deployment operations including coordinate timely and accurate processing of medical records to and from mobilization site.
  • Assist with and attend all IMRX AND SRP events.
  • Track and report to units MRNCO their SM appointments as identified through the eCase module of MEDCHART ensuring documentation is retrieved from appointments and placed in appropriate electronic record, as well as the SM's hard copy medical record.
  • Track and report to Case Management staff, in accordance with regulatory guidance, the time a medical / dental issue is identified until either the SM is identified as medically deployable, or the SM record is sent for a MEB / PEB to determine if medically fit for retention.
  • Ensure the SM MEDPROS data is accurate and up to date before each IMRX AND SRP to assist with early identification of and follow up of medical conditions.
  • Conduct monthly case reviews in addition to reviews after each event with the Case Management staff and DSS ensuring all data have been entered accurate.
  • Provide monthly status reports of all medically non-deployable Soldiers with number that are return to duty non-compliant, medically separated, admin closed, separated and number of profiles written.
  • Acquire and maintain Armed Forces Health Longitudinal Technology (AHLTA) read and print to file capabilities.

Required Skills, Qualifications, and Experience :

  • Bachelor's Degree in a health-related field (RN, Direct Service Worker as Non-Clinical Case Managers)
  • Three years' experience as Case Manager related to Military Health care.
  • Proficient in Microsoft Word, Excel, PowerPoint, and Outlook, and proficient with the use of modules within MEDCHART and MEDPRO
  • Be a US Citizen and possess a valid driver's license.
  • Successfully complete a government background check.
  • Comply with employer's work standards and policies.

    Physical Requirements

  • Must be able to sit at computer for extended periods of time.
  • Must be able to lift at least twenty pounds.
