Connecticut Licensed Primary Care Physician

A company is looking for a Primary Care Physician for a remote position.

Key Responsibilities

Co-manage patients with their assigned primary care physician

Educate patients on appropriate treatments and care plans

Manage patient follow-ups for chronic care programs

Required Qualifications

3+ years of post-residency work experience

Active Connecticut state medical license, IMLC preferred

Board-Certified in Family or Internal Medicine

Experience in Acute Care or Urgent Care settings

Prior experience with EPIC EMR
