A company is looking for a Telephonic LPN - Care Coordination (Full Remote).

Key Responsibilities

Conduct telephonic outreach to coordinate care, admissions, and services for members

Support transitions of care for members entering or discharging from long-term care or rehabilitation settings

Gather and review clinical documentation and assist with prior authorization requests

Qualifications

Active, unrestricted LPN license in the State of New Hampshire (required)

Minimum 2 years of nursing experience in long-term care, home health, or care coordination

Familiarity with New Hampshire Medicaid processes preferred

Proficiency in EMR systems and care coordination technology

Ability to work independently in a remote setting
