Key Responsibilities :
Provide exemplary case management services by completing all required documentation accurately and in compliance with Payer, State, and COA standards.Assist in marketing and sharing information about available services to new referrals.Engage with clients, colleagues, and community members to answer inquiries, provide assistance, and address any concerns.Maintain regular contact with families, providing support and documenting interactions as necessary.Facilitate connections between families and essential community resources, ensuring follow-up on referrals for ongoing support.Conduct meetings with clients weekly or bi-weekly, maintaining consistent engagement for up to 12 months.Participate actively in weekly Treatment Team meetings to collaborate on client care.Serve as a key liaison with other professionals and agencies involved in client support.Develop recovery support plans with individuals within the first 30 days of service referral.Provide on-call support for clients as per program specifications, ensuring help is available 24 / 7.Ensure compliance with all regulatory requirements at the state level.Qualifications :
Bachelor's degree in a human services discipline (Social Work, Psychology, Sociology, Counseling, Criminal Justice, etc.) is required, with experience in case management or community resource positions, particularly with children and families.Provisionally credentialed as a PCSS in Mississippi with eligibility for full credentialing as a CSS in Mississippi.Benefits of Joining Our Team :
