Residential Assoc Clinician

Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. We are seeking a qualified candidate interested in joining our Residential Service Group Home team at our Wilson Road location in Wilmington, Delaware, 19803.

Work Schedule : Weekends, total 16 hours per week, and 8 hours shifts

We are pleased to confirm we can accommodate your new hire notice, allowing you to attend the first five-day, Monday-Friday orientation schedule, day shift at our Philadelphia location. Mileage reimbursement will be provided for your attendance at new hire orientation.

The Residential Associate Clinician will provide coordination and guidance, counseling, and encouragement to participants as they transition from residential services to a more independent environment. The Residential Associate Clinician will also implement and document participant's progress in accordance with individualized rehabilitation plans and ensure a safe, healthy and secure environment which promotes growth and development. Some additional responsibilities would include the following :

• Complete the initial and annual participant assessments and develop the participant's recovery plan from a person-centered perspective.
• Assist with the presentation in case conference for assigned participants.
• Responsible for the care and maintenance of case records for all assigned participants.
• Act as a liaison and consult with community agencies and families to maintain coordination in the recovery process.
• Provide ongoing assessment of participant's mental illness symptoms and their responses to recovery to ensure that immediate and appropriate interventions are provided.
• Participate in staff planning, training and internal / external administrative meetings.
