Group Facilitator – TBS & IOP (Nursing Home Setting)
Location : Various partner nursing homes throughout Cleveland, Ohio

Job Summary :
We are seeking a compassionate, experienced, and engaging Group Facilitator to lead Therapeutic Behavioral Services (TBS) and Intensive Outpatient Program (IOP) therapy groups within contracted nursing homes. This role is ideal for professionals passionate about behavioral health and skilled in working with individuals in long-term care environments. The ideal candidate will have experience with older adult populations and be able to foster a supportive, therapeutic group atmosphere.
Benefits :
Competitive hourly rate based on credentials and experience is $35,000 to $75,000 per year depending on work experience and credentials.
2 Weeks Paid Time Off (PTO) and 5 days of sick leave.

Example : CDCA+QMHS III : $35,000–$50,000 / LSW : $54,000–$75,000 annually
Responsibilities :
- Facilitate structured TBS and IOP groups in alignment with treatment plans and program standards
- Tailor group content to meet the cognitive, emotional, and behavioral needs of nursing home residents
- Coordinate with nursing facility staff and on site clinical team to ensure smooth service delivery
- Complete group notes, progress documentation, and any required Medicaid or compliance paperwork in a timely manner
- Monitor group dynamics and participant engagement, intervening as clinically appropriate
- Promote a safe, respectful, and inclusive therapeutic environment
- Participate in clinical supervision, team meetings, and ongoing training
Qualifications :
Must hold one of the following credentials : CDCA, QMHS, LSW, LPC, or equivalentPrevious experience facilitating behavioral health or substance use groups requiredExperience working with geriatric or medically complex populations strongly preferredKnowledge of TBS and IOP models and documentation standardsStrong interpersonal and communication skillsAbility to work independently in off-site locations (e.g., nursing homes)Must have reliable transportation to travel between assigned facilitiesMust be able to pass a background check and meet credentialing requirements for Ohio Medicaid billing (if applicable)Preferred Skills :
Familiarity with electronic documentation / EHR systems (training provided)Trauma-informed care training / certification upon hire or within ninety (90) days of hireFlexible and adaptive to different facility environments and multiple sites.Comfortable navigating long-term care settings, including walking or standing for extended periodsHow to Apply :
