Summary : T he Care Navigator serves as a patient advocate and liaison within the healthcare team, ensuring Medicare patients receive coordinated, efficient, and compassionate care. This role focuses on proactive patient outreach, timely care coordination, and patient support to reduce hospital admissions, improve health outcomes, and ensure smooth transitions of care across settings. Ideal candidate will have experience as a Nurse and / or degree in Nursing.

Essential Duties and Responsibilities :

Patient Engagement and Post-Discharge Support :

  • Serve as the primary contact for patients requiring additional care coordination and navigation services.
  • Educate patients and families about care plans, procedures, and available community resources.

Care Coordination :

  • Facilitate scheduling of appointments, tests, and procedures to ensure seamless care delivery.
  • Utilize tools and resources (e.g., CRISP, population health databases) to manage discharge alerts, monitor care transitions, and document all patient outreach and outcomes.
  • Collaborate with healthcare providers, social workers, and external resources to address patient needs.
  • Monitor and follow up on patient care plans, ensuring compliance and addressing any barriers.
  • Documentation and Reporting :

  • Maintain accurate, timely, and detailed records in the electronic health record (EHR) system.
  • Maintain detailed, up-to-date records of scheduled appointments, referrals, and pending tasks.
  • Generate reports on patient outcomes, care plan progress, and resource utilization.
  • Patient Advocacy :

  • Advocate for patient needs within the healthcare system, addressing any gaps in care.
  • Promote patient understanding and self-management of their health conditions.
  • Collaborate with providers to identify and escalate complex or high-risk patients requiring additional support or interventions.
  • Collaboration and Teamwork :

  • Participate in interdisciplinary team meetings to discuss patient care plans and strategies.
  • Actively contribute to quality improvement initiatives within the physician's office.
  • Education / Experience :

  • Requires an associate or bachelor’s degree in nursing, Healthcare Administration, or a related area and / or 1-3 years of experience in the healthcare field.
  • Prior experience in care coordination, patient navigation, or a related role, preferably in a physician’s office or outpatient setting.
  • Familiarity with EHR systems and healthcare documentation.
  • Competencies : To perform the job successfully, an individual should demonstrate the following competencies :

  • Software : MS Office (Word, Excel, PowerPoint, etc.), EHR systems, SQL, online portals, population health database

  • Physical Demands :

  • While performing the duties of this Job, the employee will have a combination of standing, sitting, bending, and reaching. May work at a computer monitors for prolonged periods. The employee may lift and / or move up to 10 pounds.
