Job Responsibilities Provide outpatient rehabilitation therapy services to patients based on physician referrals and treatment plans. Assess patient needs, develop personalized care plans, and modify them as necessary according to patient progress. Document patient progress, treatment interventions, and outcomes accurately in compliance with facility standards. Collaborate with occupational therapists, physicians, and other healthcare team members to optimize patient recovery.
