The Case Coordination Department at Hartford Hospital focuses on assessing patient needs for optimal care across the continuum. Our skilled Registered Nurses, experienced in discharge planning and case management, advocate for patients, supported by a robust team of administrative assistants ensuring efficient transitions to the next level of care.

Position Summary :

As a Case Coordinator, your primary goal will be to facilitate patient throughput. You'll be responsible for assessing patients within a specific inpatient population, coordinating their care, identifying risk factors, and collaborating with the healthcare team to implement effective transition planning. You will utilize our specialized case management computer applications to document and manage your patients' care, always prioritizing their needs.

Qualifications :

• Current CT RN license.
• ASN required; BSN preferred.
• Three years of acute-care and diverse clinical nursing experience, especially in medical / surgical nursing, is required; experience in case management, home care, or managed care is highly valued.
• Strong communication, negotiation, and conflict resolution skills are essential.
• Familiarity with computer applications and reimbursement systems is preferred.
• Certification in case management is a plus.
