Speech / Language Pathologist(H)
JOB SUMMARY
Specializes in diagnosis and treatment of speech, language and voice problems as related to human communication, as well as the diagnosis and treatment of dysphagia.
DUTIES AND ESSENTIAL FUNCTIONS
Overall performance rating of below indicated essential job duties :
Performs evaluation on patients referred in a timely manner and formulates diagnoses based on assessment findings.
Establishes individualized treatment plan and goals based on diagnostic testing.
Provides treatment for those communication areas found to have deficit during the evaluation.
Completes documentation according to practice setting guidelines.
Collaborates with other members of the healthcare team to identify needs and continued services once discharged from current level of care.
EDUCATION / LICENSE / CERTIFICATIONS / OTHER REQUIREMENTS
Minimum requirement :
Master’s Degree in the field of Speech / Language Pathology
Maintains active ASHA Certificate of Clinical Competence (CCC) in Speech Language Pathology, or in Clinical Fellowship Year (CFY).
Current active Speech Pathology license in the State of Kentucky and other states as required.
Achieves and maintains American Heart Association Basic Life Support (BLS)

Occasionally works near moving mechanical parts.

PHYSICAL DEMANDS
Frequently required to maintain in a stationary position (standing or sitting)
Constantly required to walk and move about around the Medical Center
Frequently required to operate a computer and other equipment.
Frequently required to use fine motor skills to handle and operate instruments / devices
Frequently required to position self in order to meet patient needs including stooping and crouching.
Constantly required to lift and / or move items up to 10 pounds.
Frequently required to lift and / or move items up to 15 pounds.
Occasionally required to lift and / or move items up to 25 pounds.
Constantly required to communicate with patients and other care team requiring talking and hearing.
Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.
