Our agency provides compassionate, patient-centered home health services throughout Morgan Hill and its surrounding area. We collaborate with physicians and interdisciplinary teams to help patients recover mobility, manage pain, and regain independence in the comfort of their own homes.
Position Summary
As a licensed Physical Therapist (PT) on our home health team, you will evaluate, treat, and educate patients to maximize their functional abilities. Youll design and implement personalized therapy programs, monitor progress, and coordinate with nurses, occupational therapists, social workers, and physicians to ensure integrated, high-quality care.
Key Responsibilities
- Provide skilled physical therapy services in accordance with physician orders and individualized care plans.
- Perform comprehensive evaluations, establish goals, and develop patient-centered treatment plans.
- Use manual therapy techniques, therapeutic exercises, neuromuscular re-education, gait training, and modalities (e.g., heat, cold, electrical stimulation).
- Document evaluations, treatment notes, and outcomes accurately and in a timely manner in the Agencys EMR system.
- Modify interventions based on patient response, goal attainment, and interdisciplinary feedback.
- Educate patients and caregivers on home exercise programs, safety strategies, fall prevention, and use of assistive devices.
- Facilitate referrals for durable medical equipment (DME), home modifications, and community resources.
- Participate in interdisciplinary team meetings, case conferences, and discharge planning rounds.
- Ensure compliance with federal, state, and local regulations, Agency policies, and ethical standards.
- Maintain current licensure, certifications, and complete required continuing education.

Employment begins upon successful completion of onboarding and continues until terminated by either party with 14 days written notice or immediately for cause.Strict compliance with HIPAA and Agency privacy policies is required; you will safeguard all patient information at all times.Non-compete and non-solicitation provisions apply for six months following termination :No solicitation or acceptance of employment from current Agency patients.
