General Description :
The Substance Use Care Coordinator is a specialist whose primary responsibility is to provide care coordination services for individuals initially presenting at a local community hospital and the Space Coast Health Center clinic. The position integrates directly with existing host hospital programs and functions as an extension of mission of the host hospital and SCHC.
This position serves as the lead in our medication assisted treatment program, which features naltrexone / Vivitrol and buprenorphine (Subutex / Suboxone). Persons served by this program is the primary population served by this position.
Key Responsibilities :
- Members of the team are required to provide services to all persons enrolled, regardless of assigned caseloads. The Care Coordinator will be considered the expert on matters related to the persons substance use referrals, case details, and treatment planning. As such, this person is required to :
- Respond to referrals from the hospital and community rapidly
- Develop a plan of care within the first 30 days in the program
- Ensure that Plan goals and referrals are documented in the care plan
- Coordinate and ensure that services are provided to all people served
- Ensure that all assessments and services are properly documented in the person's clinical chart.
- Serves as the primary case manager for persons served by the MAT program.
- Responsible for the ensuring the integrity of the clinical chart of assigned persons served by overseeing the timely and thorough completion of assessments, developing the person's master service plan with the assistance of the person served and the individual service team, and ensuring the timely completion of all assessment updates, treatment plan addenda and treatment plan reviews.
- Responsible for ensuring that all changes made to person's treatment plans are immediately reflected in the chart.
- Responsible for monitoring the clinical charts of assigned persons served for the completeness and quality of documentation, according to the program's expectations
- Responsible for establishing a primary therapeutic alliance with assigned persons served, family members, and other support network members
- Document summary of persons served progress at monthly intervals and whenever there is a significant change in status or as frequently as needed
- Responsible for advocating for persons served rights and preferences
- Provides services under the ideals of a Recovery Oriented System of Care
- Participates in evening, weekend and primary on-call rotation as needed
- Attends weekly treatment plan review meetings when on-duty
- Provides Care Coordination
- Provides supportive services - actively listening to the persons served point of view and providing practical counseling to enhance their motivation to persist with efforts to control negative emotions, to cope with symptoms, to solve problems, to plan, and to avoid and resolve interpersonal conflict
- Provides education about MAT options, risks / benefits, and other aspects of this modality
- Provides psycho-education on an individual and group basis to help persons served identify, adapt to, and control symptoms
- Acts as primary case manager for delivery of specialized interventions in which s / he has been credentialed.
- Coordinates with treatment providers outside the team when necessary to meet the clinical needs of persons served
- Provides ongoing assessment, problem solving, side-by-side modeling and coaching, skill training (e.g., explains, demonstrates, prompts, encourages, rewards), and environmental adaptations to help persons served improve their capacity for independent living
- Provides access and linkage with local agencies and providers for all types of healthcare needs
- Provides linkages to area resources which are identified through formal assessment, participant interview, and external assessments
- Coordinates Care through the treatment team for each individual participant, considering their unique skills and preferences
- Serves as a professional representative of Space Coast Health Centers and its programs in the community
- Assists SCHC staff as needed with mutual participants within the Coordinators scope of expertise
Requirements : Formal Education :
Bachelor's Degree from an accredited university or college with a major in psychology, social work, counseling or related human services field (i.e. one in which major course work includes the study of human behavior and development, counseling and interviewing techniques, and individual, group or family therapy). At least two years of experience in providing services similar to community-based treatment is required. Experience serving persons with substance use disorders is preferred.Work Experience :
