VHS is looking for a qualified Nurse Practitioner - Family Medicine.

• Description : Candidate CVs need to have the month & date on their work experience w / all gaps in employment accounted for.Includes some Chronic Pain ManagementChart closure within 72 hours Schedule = 18 PPD, minimum see 18 PPDMonday-Friday 8-5A mix of peds / adults (Will consider FM only)A decision is typically made within 7-14 daysWe will also accept DOBE will be considered if truly BE (5 years or less since residency)Active DEA needed before the startCredentialing takes 30-90 days (six weeks is common if the provider submit app. / supporting documents timely)The following immunizations are required for credentialing (before start date) : Flu Vaccine, TB Status / X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years).HCO & Vendor agree to a 30-day notice for ending all assignments.
• 1099 Pay Rate : Competitive Market Rates (W-2 Pay Packages available upon request)
