Mason Health Care Center, a small, boutique-style skilled nursing facility , is seeking a dedicated Social Services Director to lead our social services team. This is your chance to make a meaningful impact in a close-knit environment where residents’ well-being comes first.

What You’ll Do :

Lead and manage the social services team, providing guidance and support.

Oversee admission, care, and discharge planning for residents.

Provide counseling, resources, and support to residents and families.

Collaborate with nursing, rehabilitation, and interdisciplinary teams to ensure coordinated, high-quality care.

Maintain compliance with state and federal regulations, documentation, and reporting requirements.

Promote resident well-being, adjustment, and quality of life in a warm, boutique setting.

What We’re Looking For :

Licensed Social Worker (LSW) in Ohio.

Leadership experience in a skilled nursing or long-term care setting preferred.

Strong communication, organizational, and leadership skills.

Compassionate, resident-focused, and detail-oriented.
