Hospice Licensed Vocational Nurse

We are seeking a Hospice Licensed Vocational Nurse to join our team! You will be responsible for delivering high quality care to assigned patients.

Responsibilities:

· Administering Patient Care following the Plan of Care as established by the Physician and RN Case Manager.

· Keep RN Case Manager informed of patient condition after each visit.

· Reinforce education to Patient, Family, and Caregiver as established by RN Case Manager.

· Participate in interdisciplinary meetings to ensure patient needs are met.

Qualifications:

· Active current license

· Previous experience in nursing or other medical fields

· Ability to build rapport with patients and families

· Compassionate and caring demeanor

· Strong problem solving and critical thinking skills

· Ability to thrive in a fast-paced environment
