Job Summary :

Staff pharmacists fill orders for medications, monitor patient medication therapies, and provide drug information. Staff pharmacists supervise and direct support personnel. In the absence of the pharmacy director, a staff pharmacist may be required to assume the essential responsibilities and perform the duties of the pharmacy director.

Essential Duties and Responsibilities

  • Prepares and dispenses medications as per physician requests, adhering to established policies, procedures, and protocols.
  • Accurately interprets medication orders (verbal, written, and computerized) and updates computerized medication profiles.
  • Prepares and dispenses medications, including sterile, chemotherapy, and parenteral nutrition preparations, using appropriate techniques.
  • Issues controlled substances to patient care areas and maintains required records.
  • Ensures safe, appropriate, and cost-effective medication therapies according to established policies.
  • Monitors medication regimens for contraindications, interactions, allergies,

appropriateness, and therapeutic duplication.

  • Assists with pharmacokinetic consultations and drug dosing per protocol.
  • Reviews culture and susceptibility data for antibiotic appropriateness, recommending changes as needed.
  • Accurately interprets patient medical records, reports adverse drug reactions, and medication errors promptly.
  • Supports formulary compliance through therapeutic substitution protocols and rational medication therapy selection.
  • Provides clinical consultations and recommends evidence-based medication regimens and monitoring plans.
  • Offers drug information to professional staff, patients, and their families as requested.
  • Documents all clinical activities and interventions thoroughly.
  • Participates in quality improvement and medication-use review activities, including data collection and inspections.
  • Educates healthcare practitioners and participates in routine on-call duties for
  • emergencies.

  • Supervises and directs pharmacy support personnel, verifying daily activities of pharmacy technicians.
  • Position Description

  • Works independently, organizing and prioritizing work assignments to ensure timely pharmacy services.
  • Manages telephone inquiries and window requests efficiently.
  • Maintains clean and organized pharmacy areas and equipment.
  • Performs essential duties of the pharmacy director in their absence.
  • Maintains current pharmacist licensure.
  • Attends pharmacy staff meetings, orientation, education, and training programs.
  • Reviews pertinent literature and materials for pharmacy practice.
  • Completes all competency and skills assessment requirements.
  • Supports and promotes the mission and values of Covenant Health Ministry.
  • Other duties as consistent with this role.
  • Job Requirements

  • Excellent computer skills and typing required.
  • Medical terminology and transcription skills.
  • Excellent mathematical and analytical skills required.
  • Must have excellent staff, patient relation and interpersonal skills.
  • Excellent communication skills including reading, writing, speaking, and comprehendingthe English language. The ability to understanding written instructions.
  • Effective use of telephone skills.
  • Manage patient emotions and self-emotions such as fear, anger, hostility and violence.
  • Work alone or with a team and in crowded / confined spaces.
  • Hospital and pharmacy policies and procedures that pertain to drugs. State and Federal laws which pertain to drugs.
  • Education and Experience

  • 5-6 years curriculum in a college of pharmacy with a B.S. degree in Pharmacy or Pharm D. and 2000 hours of pharmacy internship.
  • Active licensure as a registered pharmacist.
  • 6-12 months of hospital pharmacy experience preferred.
  • Fifteen (15) hours of continuing education per year as needed for the licensure.
  • An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.

    Working Conditions / Physical Demands

    Requires color discrimination and accurate depth perception. Fine motor skills are required including grip strength and pinch grip. Must possess the physical and mental abilities to perform the tasks normally associated with this position that involve a combination of sitting, standing, walking, bending, stooping, twisting, and reaching. Lifting / carrying frequently up to 35 pounds, and occasionally up to 50 pounds. Visual acuity sufficient for work which deals largely with preparing and analyzing data and figures, computer terminal operation, and

    extensive reading. Some stress is related to a high level of responsibility for quality care. The ability to speak, read, write, and understand written instructions in English is required.

    Comp Range : $54.46 - $81.68
