General Summary of Duties : The Home Health Nurse (LPN) provides nursing care in accordance with the care plan established by the RN Case Manager.
Supervision : Reports directly to a Team Leader and performs tasks under the direction of the RN Case Manager. No supervision exercised.
Typical Physical Demands : Full range of body motion required, including the ability to lift patients, manual dexterity, and good eye-hand coordination. Position requires standing, walking, and ability to climb stairs, along with occasionally lifting items up to 30 pounds. Must have corrected vision and hearing within a normal range and be able to work under stress. Exposure to communicable diseases and bodily fluids is possible.
Typical Work Conditions : Work is performed in patients' homes, community settings, and office environments, with some risk of exposure to communicable diseases and toxic substances.
Essential Duties :
Provide skilled nursing services to patients and families in compliance with state and Medicare regulations, which includes :Assessing the physical and psychosocial status of the patient.
Implementing care plans and therapeutic goals with patients / families.Working with the RN Case Manager for referrals and reviewing progress toward goals.Utilizing agency and community resources as needed.Instructing patients / families on their care responsibilities.Document all services provided within 24 hours in accordance with regulations and agency policies, including communication of changes to the care plan.Maintain effective communication with the RN Case Manager, remaining adaptable to emergencies and unexpected visits.Develop and improve professional skills with a minimum of 12 hours of annual in-service training, while demonstrating a professional image and adhering to agency guidelines.Knowledge, Skills, and Requirements :
Knowledge of professional nursing theory, agency policies, and medical equipment.Ability to maintain quality control and react calmly in emergencies.Excellent communication skills and ability to build relationships with patients, families, and colleagues.Must possess valid transportation and satisfactory automobile insurance as per agency policy.Education : Graduate from an NLN-approved school of licensed practical nursing.
Experience : One year of recent medical / surgical nursing experience; additional community health nursing experience preferred.
License : Current active license for practical nursing in the State of Connecticut.
