Job descriptionShifts and schedule to be discussed at interview Shifts and schedule to be discussed at interview Weekly Pay with direct deposit or debit card Flexible Scheduling including weekends and evenings Rewards and Recognition Programs Career Growth Provide client care as directed by the Director of Nursing or RN supervisor. Assist with personal care (bathing, grooming, dressing, toileting). Help with feeding and monitoring vital signs (temperature, pulse, respiration, blood pressure). Prevent skin breakdown through repositioning and range-of-motion exercises. Observe and report changes in client’s physical, mental, or emotional condition. Support medication self-administration (reminders only). Perform light housekeeping, meal prep, bed-making, and maintain a clean environment. Ensure infection control, client safety, and adherence to care plans. Provide transport for appointments or activities and participate in client teaching as needed. Respond to emergencies and use equipment safely. CNA certification. Current physical or TB / PPD test, if applicable. Valid driver’s license with insured, reliable transportation. Ability to work independently, communicate effectively, and maintain confidentiality. Empathy, teamwork, and respect for clients and staff. Ability to travel up to 30 minutes for client care, training, or business needs. Any job offer is dependent on passing all pre-hire steps, which will be explained in an offer letter. These may include reference checks, confirming you can work legally, checking you driving record, a TB test, and a drug test.

Pay : $19 - $21 / hour
- Additional hourly compensation is available depending on your availability and shift differential!
We offer full-time and part-time opportunities —final schedules will be discussed and established during the interview process
Baraboo
(Main Location)
Every other weekend : 7 : 00 AM – 9 : 00 AM and 7 : 30 PM – 9 : 30 PM
Variable weekdays (Monday–Sunday) : 3 : 00 PM – 8 : 30 PM
Additional shifts available between 8 : 00 AM – 8 : 00 PM
Reedsburg
Hours available : 6 : 00 AM – 10 : 00 PM, Monday through Sunday
Every Sunday : 9 : 00 AM – 1 : 00 PM
Sauk City
Weekends : 9 : 30 AM – 12 : 30 PM (Saturday and Sunday)
Every other weekend : 1 : 30 PM – 5 : 00 PM
Lodi
Additional shifts available between 8 : 00 AM – 8 : 00 PM
La Valle
Additional shifts available between 8 : 00 AM – 8 : 00 PM
