• Schedule : 1.0 FTE. Clinic Hours 7 : 00am - 5 : 00pm (candidate to work an expanded schedule of 2 hours per week during 7am-5pm Clinic hours)
• Call Schedule : 1 : 7 Rotation; Call 5 : 00pm-8 : 00am - includes holidays, nights and weekends
• Practice Detail : Great Clinic environment, collegial team. This busy practice location currently has 2 MDs and 3 APPs; serving children to older adults; Provider would support 12-16 patients per day
• EMR System : EPIC
• Facility : Chilton Family Practice
• Location : Clinic - Calumet Hospital Campus, Chilton, WI

St Elizabeth Hospital is newly renovated with a state of the art 90-bed patient tower with a full complement of sub-specialists on staff (Cardiology, Pulmonary / Critical Care, Gastroenterology, Renal and Surgical Team).
