As a Community Support Specialist, you will help individuals adapt to living in their communities by creating personalized treatment plans, connecting them with resources, and fostering resilience and independence. You will use evidence-based practices and collaborate with other professionals to empower clients and their families to build strong foundations for long-term wellbeing. Key Responsibilities :

  • Provide community-based mental health services to children, adolescents, and / or adults including assessment, diagnosis, treatment planning, and intervention.
  • Collaborate with clients, families, and other stakeholders to develop and implement individualized treatment plans.
  • Utilize evidence-based practices such as Motivational Interviewing, CBT / DBT strategies, and Trauma-Informed Care.
  • Deliver behavioral interventions, including motivational interviewing, operant conditioning, and cognitive processing.
  • Coordinate care with primary care providers, psychiatrists, schools, and community organizations.
  • Maintain accurate and timely records of client progress and services.
  • Facilitate PSR group sessions aligned with treatment goals.
  • Participate in staff development, quality improvement initiatives, and ongoing training.
  • Work collaboratively with case managers, therapists, and support staff to ensure effective client care.
  • Develop and maintain relationships with parents, teachers, and other stakeholders involved in client support.
  • Perform other duties as assigned by leadership.
  • Education and / or Experience Qualifications :

  • Bachelor's degree in Psychology, Social Work, or a related human services field; OR
  • Bachelor's degree in another field plus two years of related experience; OR
  • Any four-year combination of higher education and related work experience; OR
  • Associate's of Applied Science in Behavioral Health Support; OR
  • Four years of qualifying experience in providing direct services to individuals with mental health, substance use, or developmental disabilities.
  • Additional Qualifications :

  • Knowledge of case management and rehabilitation techniques related to mental health and / or substance use disorders.
  • Familiarity with community resources and how to access them.
  • Strong written and verbal communication skills, including report writing.
  • Ability to problem solve, multi-task, and coordinate across providers.
  • Demonstrated ability to build rapport with diverse populations while maintaining professional boundaries.
