About The Position
Position Summary
The Physical Therapist is responsible for evaluating, treating, and rehabilitating patients with physical functioning disorders and / or wound care needs. The PT develops individualized treatment plans, delivers evidence-based interventions, and monitors progress to ensure optimal outcomes while adhering to state, federal, and professional regulatory standards. The therapist also provides education, support, and consultation to patients, families, caregivers, and interdisciplinary team members.
Key Responsibilities
Screen, evaluate, and rehabilitate patients with physical functioning disorders and / or wound care needs, creating individualized treatment plans in compliance with regulatory and clinical standards.
Utilize standardized assessments and evidence-based practices to guide treatment interventions.
Administer various therapeutic procedures, including manual techniques, ambulation, therapeutic exercises, modalities, wound care, and the use of supportive or assistive devices.
Assess patient progress, adjust treatment plans as needed, and document outcomes in accordance with regulatory requirements.
Provide consultation and counseling to patients, families, caregivers, and other healthcare providers regarding physical disorders and treatment strategies.
Communicate effectively with supervisors and interdisciplinary team members regarding patient progress, barriers to care, and treatment updates.
Prepare comprehensive discharge summaries per regulatory and clinical requirements.
Document all required clinical and regulatory information accurately in the electronic medical record system.
Accurately record treatment minutes and ensure compliance with billing and regulatory standards.
Participate in patient care conferences, utilization reviews, and rehabilitation team meetings.
Obtain necessary durable medical equipment (DME) or supplies as required for patient care.
Supervise and support physical therapist assistants, rehabilitation aides, and / or students in compliance with state laws and APTA guidelines.
Maintain professional standards of practice, ethics, and conduct in alignment with APTA and state licensure requirements.
Participate in required in-services, facility trainings, and continuing education to support professional growth.

Job Requirements
Qualifications
Experience : Minimum 1 year of PT experience required.
New Graduates : Accepted if Level II clinical experience completed (Level I observation only not accepted).
Setting : Long-Term Care (LTC) or Rehab experience required.
Licensure : Active Missouri PT License or Compact Licensure required.
Certifications : BLS certification preferred (if required by facility).
