What You'll Do
As a Master's Level Case Manager , you will provide direct support to middle school students (grades 6-8) at Chestnut TAG, Impact Prep, and Springfield Legacy Academy who are at risk for emotional, behavioral, or substance use challenges. You'll work with students, families, and school staff to improve outcomes, reduce risks, and support academic success.
Maintain a caseload of 15-20 youth and families, providing intensive Wraparound servicesConduct comprehensive assessments, including CANS, CRAFFT, SBIRT, and ACES, to identify strengths and needsDevelop and update service plans, ensuring family voice and youth engagementCollaborate with Bachelor's level Case Managers for risk management and safety planningFacilitate psychoeducational groups on social skills, emotional regulation, and risk reduction for substance useProvide parent education and skills training sessions throughout the yearDeliver support in school, home, and community settings, including after school hoursConsult with teachers, school staff, and community providers to coordinate supportsParticipate in IEP / 504 meetings and contribute to treatment planningTrack student outcomes, progress, and barriers while maintaining timely documentationWho You Are
Master's degree in Counseling, Psychology, Social Work, or a related field requiredCANS-MA certification required (or willingness to obtain upon hire)Experience working directly with youth, families, and substance use issues preferredStrong organizational, communication, and facilitation skillsAbility to work with culturally diverse populationsKnowledge of school-based and community behavioral health resourcesComfortable with technology, including documentation systems, word processing, and emailValid driver's license and reliable transportation requiredWe Hire for Purpose!
