RN / LPN MDS Long Term Care

Shift and Status Part time, 9 : 00 a.m. to 5 : 30 p.m., no weekends. Scope of work Responsible for accurate completion of the Minimum Data Set (MDS) and Care Area Assessments (CAAs) collaborating with Staff Nurses, Nurse Aides, and the interdisciplinary team on the assessment process and development of resident MDS / Care Plans. Position will monitor Medicare and Medicaid requirements. About The Facility Corewell Commons - Farmington Hills Our facility has 3 floors and 9 unit (3 units per floor), we are able to provide care for long term care residents and post-acute rehab patients. The first floor has 59 beds with a focus on sub-acute rehab, the second floor has 59 beds which focuses on long-term care and sub-acute rehab and our third floor is a secured dementia unit. Nurse-patient ratios vary depending on the shift and the population you will be serving. We care for dementia residents, long term care and sub-acute rehab residents. With 179 beds, we accept but not limited to patient specialties, such as general rehab, neuro rehab, cardiac, respiratory and cancer. We also have various programs for residents to participate in multiple activities. • A level of knowledge normally acquired through the successful completion of a Licensed Practical Nurse Program.
• 3 years of relevant experience as Licensed Practical Nurse in a long term care setting
• LIC-License Practical Nursing (LPN) - STATE_MI State of Michigan Preferred
