Job Description :Team Wellness Centeris seeking anenergetic, caring, and well-organizedCommunity-Based Clinical Therapistto join our Assertive Community Treatment (ACT) team. As an ACT Clinical Therapist, you will be responsible for providing essential services to individuals with the most serious forms of mental illness.You will spend the majority of your time in the community, helping members navigate todays complex healthcare system by offering the tools, resources, and support they need for recovery. ACT is a
- multidisciplinary, person-centered
- care model designed to maintain stability and foster independence, and as part of the team, you will collaborate on cases ranging from depression to severe mental illness and substance use disorders.
- Responsibilities :
- Provide individual and group therapy to members and their families
- Develop and implement treatment plans in collaboration with the ACT team
- Support members in accessing essential services (housing, healthcare, etc.)
- Promote wellness, recovery, and independence through therapeutic support
- Maintain accurate and timely clinical documentation
- Participate in team meetings and case reviews to ensure a coordinated approach to care
- Qualifications :
- LLMSW, LMSW, LLPC, or LPC
- (required)
- CAADC
- or development plan (required)
- Completion of
- ACT 101
- within 6 months of hire (required)
- Ability to work independently and spend a majority of time in the community
- Strong communication and interpersonal skills
- Motivated self-starter with excellent rapport-building abilities
- Valid drivers license and current auto insurance (
- must be able to transport clients
- Commitment to trauma-informed, recovery-oriented care.
