Patient Care Coach

A company is looking for a Patient Care Coach.

Key Responsibilities

Provide one-on-one mentoring and coaching to patients on device use and adherence

Monitor patient adherence and usage data, providing timely feedback and interventions

Coordinate with internal teams and external partners to resolve patient needs and ensure compliance

Required Qualifications

High school diploma or equivalent required; Associate's degree preferred

One year of work-related experience in healthcare or related fields required

Experience in health care administrative roles, customer service, or billing preferred

Foundational knowledge of chronic conditions and associated therapies

Ability to work flexible hours as needed
