The In-House Therapy Team at Shore Gardens Rehabilitation and Nursing Center , a premier long term skilled nursing facility located in Ocean County, New Jerseyis actively looking for a caring, dedicated Physical Therapist to provide quality care to its residents . The Physical Therapist evaluates and treats patients / residents, communicates with their families, physicians, and other health team members, and maintains all necessary documentation of services in the patient's medical records. Physical therapy services include : treatment planning & implementation, discharge planning, testing patients' physical abilities, analyzing rehabilitation goals, and consulting with the rehab team and other health team members to coordinate the most effective treatment program.

PT Duties Include :

• Evaluate Residents conditions & develop appropriate physical rehabilitation plans.
• Help Residents improve their mobility.
• Relieve Resident's pain using modalities, hands-on therapy, therapeutic exercises & stretching.
• Teach Residents how to use physical therapy equipment.
• Help Residents through their tasks and monitor their performance.
• Maintain Resident confidence & protects the facility by keeping information confidential.
• Complies with federal, state, and local legal and professional requirements
• Contributes to team effort.
• Submit all billing logs and necessary information on a timely basis.
• Supervising up to 2 PTA's and cosigning all notes
