Job descriptionNationalcertification in at least one of the following specialties : Family NursePractitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult NursePractitioner, Gerontological Nurse Practitioner Active,non-probationary state Nurse Practitioner License 1+year direct primary care experience as a Nurse Practitioner ActiveDEA license Provisionof exceptional primary care Conductoffice visits for routine and acute issues, and home visits as needed AdministerAnnual wellness visits and health risk assessments Carecoordination with other providers, specialists, testing facilities, andagencies Populationhealth leadership, in coordination with the Care Team Assistingthe care team with phone triage and outreach Educatingpatients on their health conditions, care plans, and treatments

Weare seeking Full time Nurse Practitioners for a primary care opportunity at ourOutpatient clinic in Woonsocket, RI. The schedule isMonday-Friday 8a-5p; no nights and no weekends.

- CompetitiveSalary (Ranges from $110,000 - $165,000 based on experience and location) withquarterly bonus based on quality metrics
