Job Description :

Work under the supervision of the physical therapist.

Accept referrals from physicians for patients with physical disabilities.

After evaluation by a registered physical therapist, treat the patient as prescribed by the physician.

Record new patient s information and progress notes and be able to summarize information from the therapist s evaluation.

Obtain physician prescriptions.

Communicate all new patients information to business office.

Maintain good relations with the referring physician.

Supply all billing information to the billing department.

Perform other duties as assigned by the physical therapist.

Attend all scheduled staff meetings and all other meetings upon request.

Assist the staff as deemed necessary

Qualifications :

Graduate of a 2 year accredited CAPTE college level program approved by the American Physical Therapy Association.

Licensed by the State of Michigan
