Home Care RN Per Diem

Assisting Hands Home Care® is seeking a Registered Nurse (RN) in the Toms River, New Jersey area. This is an opportunity for someone who has a passion for helping seniors and a proven track record of providing exceptional care.

Job Description : The RN Care Coordinator will be responsible for overseeing delivery of all companion and personal care services delivered by the agency for assigned clients. This includes an initial Nursing Evaluation visit and establish a Plan of Care. 30 day follow up calls, and 60 day visits as mandated by New Jersey regulatory standards.

The RN will be compensated per visit in the following increments :

• $125 for initial assessment / POC
• $15 per 30 day follow up call (these take approximately 5-7 minutes)
• $75 for 60 day visit / Caregiver Supervisory Visit

Documentation is all electronic and can be done on any mobile device.
