Columbus House, Inc. is seeking a Family Triage Coordinator for a seasonal full-time position from December 1, 2025, to TBD. Under the direction of the Manager of Case Management Services, the Family Triage Coordinator will provide program coordination and supervision to residential staff.
The purpose of this role is to provide direct supervision and program management, training, and professional development to Resident Support Specialists at the Family Triage program, which has a capacity of 40 people (mothers and children total). Families may have struggled with substance abuse, mental illness, and homelessness.
Duties and Responsibilities :
- Create strength-based person-centered approaches
- Provide linkages to clients who need social, mental health, educational, vocational, recreational, transportation, spiritual, and / or substance use services
- Coordinate and collaborate with other agencies and social service providers
- Enforce program rules and supervise resident chores and bi-weekly house meetings
- Train and empower supervisees to be effective in the completion of tasks
- Attend weekly and monthly agency, program, and network meetings
- Maintain and update all required charting and notes as required by Columbus House, and other related duties as assigned to support the operation of Family Triage
- Enter and maintain Communication Log and any other tracking sheet created for the Family Triage
- Hire, train, and supervise all residential program staff, providing all necessary oversight and supervision and ensuring consistency and continuity of services rendered
- Monitor Residential Staff scheduled Relias Online Training monthly
- Provide monthly supervision to the Family Triage Staff
- Complete the Residential Staff's Annual Employee Performance Evaluation
- Provide supervisory coverage to the Family Triage Program
- Responsible for the completion of reports as needed
- Responsible for the provision of a safe and healthy environment for the Families and Staff at the Family Triage
- The coordinator will be on call and provide supervision and guidance as needed
Qualifications, Knowledge, Skills, and Abilities :
Bachelor's degree in human services (preferable) with a minimum of 2 years' experience working in human services or related fields or five years' experience as a case manager working in human services or related fieldsExcellent organizational and time management skills with a deadline-oriented mindsetStrong written and oral communication skills employing the English languageProficient in Microsoft Office and other common desktop applicationsAble to effectively use the local network of substance abuse, mental health, and social services providersFamiliar with criminal justice and able to engage, manage, motivate, and supportFamiliar with mental health, substance abuse, and dual diagnosis issues and group dynamicsComplete knowledge base of all aspects of the delivery of person-centered case managementAble to meet all documentation requirementsKnowledgeable about crisis intervention, boundaries, confidentiality, universal precautions, harm reduction, trauma-informed care, and motivational interviewingValid driver's license with a good driving historyColumbus House, Inc. is an equal opportunity employer.
