We are seeking a full-time Community Health Worker (CHW). The CHW will serve as the link between People with Justice Involvement (PWJI) - particularly those transitioning from prison back into the community. This position will connect PWJI to healthcare and reentry support services to address their complex medical and social needs to improve overall health and well-being. Additionally, the CHW will serve as a mentor to Residents at Marcus Garvey II . This position is 9-5 Monday- Friday
Supportive housing residence, Marcus Garvey, for individuals returning from incarceration. Marcus Garvey is located in the Brownsville neighborhood of Brooklyn and has 88 units of supportive housing for adults 50 years of age or older who are reentering the community from incarceration and who would otherwise be homeless or unsuitably housed. The services provided by Osborne will be designed to address all the aspects of life that a formerly incarcerated person needs to rebuild upon reentry to the community.
