Meets or exceeds productivity as established by the administrator.Prepares appropriate document for mileage, visits and or time, and submits to the office for processing and administrative review daily.Report to the patient's case manager any changes in condition, problems, complications, special needs, patient caregiver compliant or any other circumstances concerning the patient family or home environment.Reports to work as scheduled, verifies Home Hospice Assignment prior to each visit and provides care in accordance with Agency policy and procedures.Safely provides personal care for patients, following the Aide Care Plan to include : Bed bath sponge bath tub bath tub bath with bench or chair. Assist with and provide patient privacy, provide comfort measures, shaving, skincare, perineal care, catheter care, hair care, oral hygiene, nail care (cleaning, no cutting or clipping of nails), foot care (no cutting or clipping of nails). Safely assist patients with positioning, transferring and ambulation, assist with bedpan or urinal, bedside commode or toilet. Change and straighten linen, assure patient's oxygen supply is maintained (if applicable) during visit, and straightening of the immediate environment (patient's area).Adheres to all Infection Control guidelines in accordance with Agency policy and procedures.Any other special assignments as requested by supervisor.Assist in maintaining safe, healthy environment for patient and family.Assist in orientation of new CNAs.Assist in the after hours, on call, and weekend rotation schedules for patient visits.Completes CEU's as required by state and federal regulations. Completes net-learning as required by Agency policy and procedure.Demonstrates and develops commitment to person professional growth.Demonstrates maintenance of confidentiality of patient information.Handles the personal property of patients and families with care and respect.Maintain accurate and complete clinical and personnel records per policy and procedure.
