Responsibilities
What you will do : (responsibilities listed are included but not limited to)
Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their familiesSets measurable objectives that are formulated in conjunction with the rehab teamAssists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problemsInstructs / counsels’ patients and families in treating and coping with social and emotional response connected with illnessesParticipates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needsIdentifies and assists the patient and / or family to utilize appropriate community resources to achieve measurable objectivesQualifications
What you’ll need :
Master's Degree in Clinical Social Work (LCSW)Current Licensed Clinical Social Worker Licensure in the state of practiceMinimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosisCPR certification requiredAbility to lift 50 poundsMoving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety proceduresDuties require fine motor skills, visual acuity, and walking / standing for extended periodsAdditional physical requirements include pushing / pulling, bending / stooping, reaching, kneeling, and positioning frequently at times
